Serotonin syndrome results from excessive serotonergic activity in the nervous system.
It can occur when serotonergic medications are combined, doses are increased, or interactions cause excessive serotonin activity.
Medications associated with serotonin toxicity can include certain:
- SSRIs
- SNRIs
- MAO inhibitors
- Tricyclic antidepressants
- Other serotonergic medications

Classic Findings of Serotonin Syndrome

Mental Status Changes

What Is Neuroleptic Malignant Syndrome?
Neuroleptic Malignant Syndrome is a rare but potentially life-threatening reaction associated particularly with dopamine-blocking medications such as antipsychotics.
NCLEX questions may describe a patient who recently started an antipsychotic or had a significant dose change.
The patient then develops:
- High fever
- Altered mental status
- Autonomic instability
- Severe muscle rigidity

Associated with certain:
Anesthetic agents and succinylcholine
Malignant hyperthermia typically occurs in relation to anesthesia and involves rapidly increasing metabolic activity, muscle rigidity and hyperthermia.
Medication history is therefore crucial.
NCLEX Prioritization
Suppose four patients are presented:
One reports mild nausea after starting an antidepressant.
One has a mild hand tremor.
One taking an antipsychotic develops a temperature of 40°C with severe generalized rigidity and confusion.
One reports difficulty sleeping.
Which patient requires immediate assessment?
The patient with:
High fever + severe rigidity + altered mental status
because these findings may indicate NMS.
NCLEX prioritization is based on the risk of rapid deterioration, not simply which symptom sounds most uncomfortable.
