Clinical Judgment and NGN in the 2026 Syllabus
Clinical judgment is not a separate subject studied only after completing
nursing topics. It is integrated throughout the entire NCLEX-RN examination.
The official framework includes six clinical-judgment steps:
Recognize Cues
identify important findings from sources such as the history, assessment, vital signs and laboratory results.
Analyze Cues
connect the findings and determine what they may mean clinically.
Prioritize Hypotheses
identify and rank the most urgent or likely client concerns.
Generate Solutions
determine appropriate expected outcomes and possible interventions.
Take Action
select and implement the safest priority response.
Evaluate Outcomes
compare the client’s actual response with the expected result.
Clinical judgment is explicitly measured through three six-item case studies—18 case-study items—and approximately 10% stand-alone clinical-judgment items, depending on examination length.
