How to use this glossary for NCLEX-RN preparation
Use the topic groups below to study medical–surgical nursing, emergency conditions, endocrine, infection, maternity, psychosocial integrity and the NGN clinical judgment model. For detailed patient exercises see free clinical judgment case studies and the Malayalam beginner guide.

Seven essential pairs: don’t confuse these terms
Hypotension versus shock
Hypotension = താഴ്ന്ന രക്തസമ്മർദ്ദം; Shock = മതിയായ ടിഷ്യൂ perfusion നഷ്ടപ്പെടുന്ന അവസ്ഥ. എല്ലാ low BP reading-ഉം shock അല്ല; എല്ലാ shock-ഉം ആദ്യഘട്ടത്തിൽ hypotension കാണിക്കണമെന്നില്ല. NCLEX reasoning: A worsening BP trend with tachycardia, low urine and confusion suggests urgent impaired perfusion.
Hypoxemia versus hypoxia
Hypoxemia = arterial blood oxygen കുറയൽ; hypoxia = tissue oxygen deficiency. NCLEX reasoning: Assess oxygen delivery, circulation, breathing and patient signs; do not treat terms as identical.
Assessment versus intervention
Assessment = രോഗിയുടെ നില വിലയിരുത്തൽ; intervention = പ്രശ്നത്തിന് ഉചിതമായ nursing നടപടി. NCLEX reasoning: New clinical deterioration often calls for immediate assessment and rapid action/escalation, not paperwork.
Side effect versus allergy
Side effect = അറിയപ്പെടുന്ന അനുബന്ധഫലം; allergy = immune-mediated reaction, potentially severe. NCLEX reasoning: New airway swelling after a drug is an emergency; simple transient nausea may have different implications.
Delegation versus assignment
Delegation = law/policy-defined additional task transfer; assignment = routine role-authorized duties വിഭജിക്കൽ. NCLEX reasoning: Actual nurse practice act matters; don’t memorize a universal UAP rule.
Recognize versus analyze
Recognize cues = findings കണ്ടെത്തുക; analyze cues = findings തമ്മിലുള്ള ബന്ധം മനസ്സിലാക്കുക. NCLEX reasoning: Isolated BP low versus BP low plus tachycardia and confusion.
Compensation versus correction
Compensation = acid-base disturbance-ന് ശരീരത്തിന്റെ മറുപടി; correction = കാരണത്തെ ചികിത്സിച്ച് പ്രശ്നം നിയന്ത്രിക്കൽ. NCLEX reasoning: Near-normal pH can still have high CO₂ and high bicarbonate.
Eight original self-test questions
Practice 1. Patient has new stridor and SpO₂ 86%. Which word identifies an airway warning?
Answer and rationale: Stridor — ശ്വാസവഴിയിലെ അപകടസൂചന; prompt emergency airway assessment.
Practice 2. K⁺ 6.9 mEq/L with QRS widening. Which term?
Answer and rationale: Hyperkalemia — potassium elevation with potential life-threatening cardiac effects.
Practice 3. A post-delivery patient has a boggy fundus and heavy bleeding. Which terms?
Answer and rationale: Uterine atony and postpartum hemorrhage — requires urgent assessment and obstetric response.
Practice 4. Patient has pH 7.25, low HCO₃⁻ and compensatory rapid breathing. Which acid-base process?
Answer and rationale: Metabolic acidosis — interpret with full ABG, clinical history and compensation.
Practice 5. An RN asks UAP to record stable intake but gives no report thresholds. Which concept?
Answer and rationale: Delegation/right directions — clarify task expectations, escalation and supervision.
Practice 6. A patient’s HR climbs 90 to 132 with BP falling. Which NGN item is useful?
Answer and rationale: Trend question — combine measurements over time, not one isolated number.
Practice 7. New mental status change appears during heparin therapy. Which priority?
Answer and rationale: Assess patient and potential bleeding or other cause; don’t assume a benign side effect.
Practice 8. Patient with fever and hypotension becomes confused. What nursing concept?
Answer and rationale: Sepsis/possible shock with clinical deterioration — prompt ABC and team assessment.
Suggested Malayalam-to-English learning method
- Choose one organ system per day and learn 10–15 terms with pronunciation and English spelling.
- Read a short patient case and underline terms that are actual changes from baseline.
- Explain each finding in Malayalam, then restate the nursing concern in English.
- Link medicine terms to actions: potassium → cardiac rhythm; opioid → respiratory monitoring; oxygenation → ABCs.
- Review which terms can be confused, particularly hypoxia/hypoxemia, assessment/action and assignment/delegation.
- Apply terminology to safe NCLEX rationales rather than memorizing a vocabulary list without context.
Five frequently asked questions
Why use Malayalam explanations for English nursing terms?
Malayalam can clarify meaning, but NCLEX-RN candidates must learn the original English terms and how they appear in patient records.
Is this dictionary a replacement for the official NCSBN test plan?
No. It is an educational companion; the official 2026–2029 Test Plan defines exam categories and clinical judgment.
Do all English medical terms have an exact one-word Malayalam equivalent?
No. Several require a short explanation instead of literal translation; the table provides practical meanings rather than legal clinical definitions.
Are the reference values and treatments listed here complete?
No. For detailed ABG, electrolyte and medication safety, use the linked clinical guides and applicable clinical protocols.
How should nurses practise these words?
Read a short patient case, identify five important English cues, explain them in Malayalam and then state the nursing priority in English.
