Sprain, ligament injury and initial care — NCLEX-RN learning guide

A sprain is a ligament injury, ranging from stretching to partial or complete tear. RICE (Rest, Ice, Compression, Elevation) is a commonly taught approach to short-term symptom management; clinical examination determines whether imaging, protection, rehabilitation or further treatment is needed.

മലയാളത്തിൽ പഠിക്കേണ്ടത് | Malayalam lesson summary

എന്നാൽ severe pain, deformity, inability to bear weight, numbness, cold limb എന്നിവയുണ്ടെങ്കിൽ fracture അല്ലെങ്കിൽ neurovascular injury ഒഴിവാക്കാൻ prompt evaluation ആവശ്യമാണ്.

Video learning recap (caption-informed, not verbatim)

  • 00:00–00:39: Ligaments and causes of sprain
  • 00:39–01:12: Pain, swelling and movement limitation
  • 01:12–01:36: RICE explanation and severe-injury assessment

NCLEX-RN clinical reasoning: key takeaways

  • Assess neurovascular status, deformity, functional limitation and pain.
  • Avoid excessive compression and prolonged immobilization without a clinician-directed plan.
  • Follow injury-specific guidelines and graded rehabilitation; RICE alone does not rule out fractures or major tears.

Further study: PEACE and LOVE alongside RICE

RICE is a familiar acute symptom-management mnemonic taught in the video. For broader soft-tissue recovery, a British Journal of Sports Medicine editorial describes PEACE (Protect, Elevate, Avoid anti-inflammatory modalities, Compress, Educate) and LOVE (Load, Optimism, Vascularisation, Exercise). These are educational approaches, not substitutes for diagnosis or an individualized treatment plan.

NCLEX-RN 2026–2029 study companion: Sprain: rule out more serious injury

New educational explanation: A sprain injures a ligament, but swelling and pain can also occur with fractures or neurovascular compromise. RICE is a familiar first-aid mnemonic, while PEACE and LOVE emphasizes stage-appropriate rehabilitation. Neither framework replaces assessment of distal circulation, sensation, movement, deformity and weight-bearing ability.

Original NCLEX-style clinical judgment practice

Question: After an ankle injury, a client has worsening pain, numb toes and a foot that is cool compared with the other side. Which action is most important?

  1. Continue a tight compression wrap without reassessment
  2. Assume ordinary ligament swelling because a sprain was suspected
  3. Assess distal neurovascular status and obtain urgent clinical evaluation
  4. Delay assessment until the next exercise session

Correct answer: C. Rationale / വിശദീകരണം: C recognizes possible impaired perfusion or other serious injury. A may worsen constriction, while B and D delay essential evaluation. Treating staff should evaluate the bandage and injury promptly.

Malayalam–English quick terminology

  • Ligament — tissue connecting bones
  • Neurovascular — nerve and blood flow status
  • Rehabilitation — guided restoration of function

Two learning questions

Does RICE exclude fracture? No. Suspected fracture or vascular compromise needs assessment.

Is prolonged immobilization always recommended? No. Immobilization and gradual loading depend on the injury and professional instructions.
