Vertigo, positional dizziness and BPPV — NCLEX-RN learning guide
Vertigo describes a perceived sensation of spinning or movement. Benign paroxysmal positional vertigo (BPPV) commonly causes brief episodes triggered by position changes, but dizziness has many causes and must not automatically be labelled a benign ear problem.
മലയാളത്തിൽ പഠിക്കേണ്ടത് | Malayalam lesson summary
Vertigo spinning sensation ആണ്. BPPV-യിൽ തലയുടെ position മാറ്റുമ്പോൾ ചെറിയ സമയം spinning അനുഭവപ്പെടാം. എന്നാൽ sudden weakness, slurred speech, severe new headache, inability to walk, double vision, new neurological deficit എന്നിവ stroke പോലുള്ള central causes സൂചിപ്പിക്കാം — അടിയന്തര വിലയിരുത്തൽ ആവശ്യമാണ്.

NCLEX-RN clinical reasoning: key takeaways
- Assess onset, duration, triggers, gait, vital signs and neurological findings rather than assuming BPPV.
- Epley or other repositioning maneuvers are appropriate only for a suitable diagnosis and patient after professional assessment.
- Implement fall precautions and escalate acute focal neurological symptoms.
NCLEX-RN 2026–2029 study companion: Vertigo: BPPV versus neurological emergency
New educational explanation: BPPV typically causes brief spinning triggered by changes in head position. However, dizziness and vertigo can also reflect central neurological disease. Sudden speech changes, focal weakness, double vision, severe gait disturbance or a new severe headache require urgent emergency assessment. Repositioning maneuvers are not a substitute for evaluating warning signs.
മലയാളത്തിൽ പ്രധാന ആശയം: Position മാറുമ്പോൾ short spinning = BPPV സാധ്യത. പക്ഷേ sudden slurred speech, limb weakness, double vision, walking inability വന്നാൽ stroke rule-out urgency ആണ്; routine Epley first step അല്ല.
Original NCLEX-style clinical judgment practice
Question: A client arrives with sudden severe dizziness, new double vision and slurred speech. What should the nurse prioritize?
- Activate urgent neurological/stroke evaluation according to protocol
- Perform an Epley maneuver before any neurological check
- Provide only a motion-sickness tablet and discharge
- Assume a benign inner-ear problem because dizziness is present
Correct answer: A. Rationale / വിശദീകരണം: A responds to possible central nervous system disease. B, C and D may dangerously delay stroke assessment. The nurse should assess vital signs, neurological status, safety and immediate escalation.
Malayalam–English quick terminology
- BPPV — benign paroxysmal positional vertigo
- Diplopia — double vision
- Central vertigo — vertigo from brain/central pathways
Two learning questions
Does every vertigo episode mean BPPV? No. Duration, triggers and neurological findings guide differentiation.
Is Epley appropriate for everyone with dizziness? No. It is used for suitable diagnosed BPPV, not unexplained neurological red flags.
